No Room for Error

Focusing on human error and why people make mistakes




Designed principally for senior management and company trainers, No Roomf or Error uses a series of five maritime scenarios to illustrate the root causes of human errors.

No Room for Error uses realistic scvenarios filmed on board to illustrate the pervasive effect of human error in situations that result in collisions, personal injury, pollution, and cargo and property damage.

Traditionally, insurers have concentrated on the proximate cause causes of claims, however the UK Club suggests that 99% of claims are caused by underlying (latent) failures. The underlying, contributing causes of any incident, not so obvious, often highlight shortcomings in procedures, practices, and equipment across the whole spectrum of a shipping operation, creating the conditions in which incidents and accidents occur.

The five scenarios in the video are described in full detail. The incidents are then reviewed in terms of the latent failures, which cover eleven categories: procedures, hardware, design, maintenance, error enforcing conditions, housekeeping, incompatible goals, communication, organization, training and defences.

Described as the Club's main vehicle for reaching on board and onshore personnel across the entire shipping operation, No Room for Error aism to get past the proximate cause and look at the latent failure and chain of events.
