The Club has produced a series of award-winning educational training videos based on common, real life shipping accidents. Analysis of these incidents, which have caused death, injury and ship damage, serve as lessons to Members and prompt the critical question: Could this happen on my ship?
The video series features incidents involving injury, pollution, cargo claims and navigation, all gleaned from the thousands of Club claims along with five decade of extensive research into loss prevention issues and risk. Supported by an extensive catalogue of written case studies, these videos identify key lessons learnt to help members avoid similar incidents.
Incidents during lifting operations show a pattern of repeating events, with the UK Club’s work on Incident Prevention showing that an appalling number of these incidents result in life-changing injuries and tragic loss of life
On a loaded bulk carrier, a bulkhead stool void space was due for periodical internal inspection. The C/O instructed the bosun to open the main deck lid to the vertical access trunk and place a small ventilation fan over the opening.
At the time of the incident the subject vessel was alongside loading bulk cargo. At the same time heavy fuel oil was being loaded from a bunker barge. Weather conditions were good but it was very cold (-20 C°) and there was thick ice and snow on deck.
The Club is aware of a number of accidents occurring during the making fast or letting go of a tow. In another case, a carpenter was killed after being struck on the neck by a messenger line jumping off a warping drum.
